Continuing Student REGISTRATION is the process by which we update personal information such as address, email address, phone numbers, emergency contacts, etc. You will be required to Correct, Sign and Date the registration document provided.
Please contact site personnel to communicate any changes regarding your student’s demographic information (Student, Parent, or Emergency Contacts). - Birth certificate and/or passport
- TWO current proofs of residency
- Updated immunization records
- Most recent report card and/or transcript from previous school
- Most recent standardized state test results (used to assist in math placement)
- GATE test results or results from similar program
- Most recent IEP, 504, ELL
